Referral Form
Thank you for your interest in our assessment services. We are dedicated to providing thorough, evidence-based evaluations to support the needs of children and young people. Please note that due to availability and the current scope of practice, ongoing intervention (therapy) is not offered or provided.
Our Assessment Scope: We provide comprehensive assessments for children and young people from ages 4 to 18. Our clinical expertise includes:
Neurodevelopmental Assessments: Autism Spectrum Disorder (ASD) and Attention-Deficit/Hyperactivity Disorder (ADHD).
Cognitive & Learning Assessments: Identifying learning disorders and cognitive profiles.
Complex Presentations: Assessments for Fetal Alcohol Spectrum Disorder (FASD), Disruptive Mood Dysregulation Disorder (DMDD), and other mental health conditions.
Please note: Autism assessments are available for ages 4 and above, and cognitive assessments for ages 6 and above.
The Assessment Journey: Our process is thorough and designed to provide a deep understanding of each child’s unique profile. Because we prioritise clinical accuracy, the overall process typically takes 1 to 3 months.
Referral & Questionnaires: Once we receive your completed referral form, we will send out questionnaires to parents and educators.
Assessment Sessions: After the questionnaires are returned, we will schedule an initial consultation followed by 1–2 assessment sessions.
Reporting: Your final report will be completed 4–6 weeks after our final session.
Note: To ensure the best clinical outcomes, we do not conduct sibling assessments concurrently. We recommend starting with the oldest child or the most complex case first.
Funding & Fees
NDIS: Plans can cover face-to-face assessments and the associated psychological functioning report. However, given the NDIS does not cover diagnostic assessments, we offer these for a private fee of $250 ($60 for Health Care Card holders).
Current Availability: We are currently accepting new referrals for assessments. Clients are prioritised based on the completion of parent and educator questionnaires.
